Terms and Conditions

I understand that my student may participate in physical activities such as, but not limited to, those held during Game Time. As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any legal liability, the Milton Seventh Day Baptist Church and any persons involved in Collision Student Ministries.
In the event of any emergency that requires medical treatment for the above named students, I understand every effort will be made to contact me or my emergency contact. However if I/we cannot be reached, I give my permission to the Collision Student Ministry volunteers to secure the services of a licensed physician to provide the care necessary for my student’s well-being. I assume responsibility for all costs connected to any accident or treatment of my child.
I understand that at Collision or other Collision related events, my student may be photographed. I agree to allow my student’s photo, video or film likeness to be used for any legitimate purpose by the leaders, organizers or church members, including but not limited to social media and newspapers. When an identification is made, only the first name of the student will be used unless a parent is notified otherwise.
On Wednesday nights during our regular Collision meetings and during special events there will be activities that require transportation. I give permission for my child to be transported by Collision staff.
