If you do not have a printer, printed copies of these forms are available in the church workroom.

Please return your completed, signed copies of the forms to Brittany’s mailbox in the church workroom or scan the signed form and email it to churchoffice@miltonsdb.org.

• If you are doing your annual review, please print and sign the Staff and Volunteer Agreement form.
• If this is the first time you have done Safety in Ministry Training, please print and sign:
  • Staff and Volunteer Agreement form
  • Authorization for Background Check
  • Screening Form for Volunteer Workers
