Then, be sure to fill out and send in the forms as indicated.

After watching the video, print and fill out the forms as instructed below.
If you do not have a printer, printed copies of these forms are available in the church workroom.
Please return your completed, signed copies of the forms to Pastor Liz’s mailbox in the church workroom or scan the signed form and email it to pastorliz@miltonsdb.org.
- If you are doing your annual review, please print and sign the Staff and Volunteer Agreement form.
